Patient Forms

We are honored that you have chosen us to care for your foot and ankle needs. Visiting a new doctor's office for the first time is not always an easy process, and it can frustrating at times. Our goal is to make you feel comfortable and welcome. To help you prepare for your first visit, we offer some of the following suggestions:

  • When calling to schedule an appointment, try to have your insurance policy information available.
    • Our staff will check your eligibility before your visit, to prevent confusion on the day of your appointment.
       
  • Read through our Patient Forms, available on our website by clicking HERE.
     
  • Fill out your registration paperwork prior to your appointment. There are two ways to complete your paperwork:
    • Visit our Patient Portal (recommended).
      * See below for more information about the Patient Portal.
    • Print the forms from the website, fill them out, and bring them to your appointment.
       
  • Arrive 10-15 minutes prior to your appointment time.
    • Any additional insurance verification, registration, or medical history paperwork will be completed during this time.
    • Make sure to bring your insurance card, drivers license, and prior authorization/referral information (if required).
       
  • Visit our Patient Portal.
    • Our Patient Portal gives you access to your health information. It gives you the ability to see your medical records, test results, and appointment summaries.
    • The Patient Portal also allows you to register for your appointment, and complete your new patient forms from the comfort of your own home. This allows you to complete the forms at your own pace, and it will save you time in the waiting room, when you arrive to the office!
